Can you provide me a copy of any power-point presentations because my disability prevents me from writing with speed. My email address is firstname.lastname@example.org
I would also like to request the ppt - email@example.com
Hi Everyone, We will be sending out the powerpoint and the recording tomorrow to everyone who registered for this presentation. The recording will go up on our website as well.
You are welcome!
how am I eligible for self-determination; I currently reside in an eight-bed facility in the Inland Empire. I'd like to see if I'm a candidate for this program. Please send all pertinent information to my email address which is firstname.lastname@example.org. Thank you for all your assistance with my request. I used to be a consumer of independent living, but after the state changed the criteria I have been the group home system
my email address is email@example.com
Kim- Individuals living in a group home can still be eligible. Living in a place like a skilled nursing facility would require you to move out of the skilled nursing facility.
Kim- You can be in SDP and live in a group home as long as it meets the Home and Community Based Services (HCBS) requirements.
Here is a link to frequently asked questions. It has a dropdown for Eligibility https://www.dds.ca.gov/initiatives/sdp/frequently-asked-questions/
how is self determination different from in home support services?
In Home Supportive Services (IHSS) is a county operated program that is considered a generic resource by the regional center.
More info about Self-Determination
Self-Determination is a program that lets you choose your own provider if you need support at home and your community that is extra from IHSS.
What kinds of services can you pursue under self-determination?
Question from Randa (I want to make sure we get everything answered) If you switch to self-determination, does that change respite services and the medi-cal waiver?
In traditional, if you needed additional help at home or in the community, you would choose an ILS (Independent Living Service) or SLS (Supportive Living Service) vendor provider off a list. In Self-Determination, you don't have to choose a vendor to help you at home.
Rand- No, going to SDP does mean you have to change your respite. What would be important is to make sure that your provider will accept payment from a Financial Management Service provider instead of being paid by regional center accounting.
SDP is a waiver program and services you received from Medi-caid waiver can transfer over, including deeming for MediCal under 18. Hope that helps.
Yes, it does. Thank you!
Randa- I caught a typo- You do NOT have to change your respite provider as long as they accept payment from FMS. :-)
can you email information to firstname.lastname@example.org because writing is very difficult
How do you get recreation classes paid? How do we get a company to partner with SDP
Randa, Hire a good, experienced Independent Facilitator. they can help
I agree with Cindy. Once you are in SDP, you can work with your FMS to help set-up your providers to be paid.
Most providers are good with being paid by the FMS. A good IF can help you navigate all the different steps.
Is an independent facilitator part of self determination? Or is that person an outside person that we have to pay for them to advocate?
It is someone that can help you get into to SDP. Regional Centers can pay your IF up to $2,500 to help with the SDP transition.
Ongoing IF support would be paid for out of your SDP budget and through your Spending Plan.
Randa: Funding is provided for an Independent Facilitator to transition you into SDP. It is an outside person
thank you! This information is really needed to help switch
From Margaret: We have trouble finding staff. Any tips?
Where can you find a list of independent facilitators?

Question for Tina: Is your live-in caregiver your IHSS provider too? The IHSS hours are different from the live in caregiver hours?
Margaret- Care.com, colleges nearby (CNA students), local ads, Facebook ad, local paper...
Joe Hernandez | SCDD
SDP Service Codes: https://www.dds.ca.gov/wp-content/uploads/2019/05/SDP_Service_Codes_02142019.pdf
SCDD has a list of IFs
I have a question
Thank you for all of the helpful information.
Another Source for IFs: https://www.thecasdpnetwork.org/the-independent-facilitator-network/
My email address is email@example.com
Kim- Did you want me to email you the information on eligibility?
From Shellie: Would an IPP need to be updated to fit the person centered plan? Will the Regional Center state services are not in the IPP prior to the transition?
how about Illinois servers
my oldest daughter is sever autistic and youngest is mild autism
Translation of Delfina's question: My son is 22 years old, and my coordinator says it's not worth self-determination, because he has only respite, so he is in the IPP.
Could you provide me some additional materials or resources so I may pursue this service / program through my particular Regional Center
Thank you for the great information. I must sign off, now.
Delfina: NOT TRUE. There could be other unmet needs. Please talk with an Independent Facilitator.
my oldest daughter doesn't like to go outside or be by people to interact
Shellie: Yes, updating the IPP happens to reflect items in your PCP and services you want to fund in your Spending Plan.
Shellie- Your regional center should not change your IPP to state that they will not fund services before you transition.
But, an updated IPP will happen? We just went through his IPP.
how about services for Illinois
HCBS Final Rule https://www.dds.ca.gov/initiatives/cms-hcbs-regulations/
Shellie- Yes, if services and goals look different then the IPP would need updated.
My son loves trains. Would taking a train to Disneyland be a service that could be covered? I would like him to learn how to use public transportation.
1 minute Survey about tonight's presentation - https://www.surveymonkey.com/r/TNQY7DZ
Shellie: Learning to use public transportation could be a service. A Disneyland ticket...no
There are requirements and restrictions that apply to being a paid IF.
Very important to find an experienced IF.
can you please email the resources for self-determination/qualifications for this program because I have difficulty writing with any speed.
Thank you! That’s perfect!
If you want help getting all the way through the transition process, be careful that you do not spend the entire $2,500 allowed on a Person Centered Plan and only a plan.
IFs often attend local SDAC meetings
From Margaret: If our plan shows $20 or $19 per hour for staff and I can't find anyone willing to work for that, can I pay more as long as I don't go over the budget/spending plan?
Margaret: I would talk to your FMS and have them work with you to determine what you can pay so you get the coverage you need and not go over budget. Including things like overtime.
Thank you panel, great topic and hope you have a great rest of your evening!
This has been very inspiring. It makes me hopeful that my Son can have meaningful experiences and a good quality of life.

From Delfina (translation): My child has very severe needs, occupies help to walk, eat, dress, in everything occupies help, intellectually, is not functional, still, the program applies to my child, he is 22 years old
From Kim: could you give me Self-determination criteria for this service. I used to be a consumer of SLS, but I had heath complications so I now live in a residential facility; this servicve souds very appealing to me.
Are service coordinators worried they will lost their jobs?
I live in Illinois
Dr Doug Perkins
CSCs should not be worried about their jobs being lost. No reason, I'm aware of the Rc system.
Love the Panel!!!!
Thank you for this workshop. Very helpful.
Kim, this program is specific to California. However each state may have their own program. I recommend checking with your local social services department or if there are regional centers there.
Thank you so much!!
We only have respite service in traditional service. With Self determination program, we have more services. There’s are unmet needs and new needs you can add in SDP.
Thank you for you time😉
