Another query: I am applying as an S38 as a returning to practice. I'm currently going through recognition as I trained in UK. I've seen the requirements for return to practice but what will I need to submit for proof I have done the return to practice?I mean submit to CORU

Some of my documents have my shortened first name and others my longer name but my surname is the same. Will this be a problem?

From Lee: Regarding name variation: This should not be a problem, you can declare this when making your application to clarify the situation. If the team are unclear they will contact you for clarification. It is a common occurrence and it should not have a negative impact.

Question for Lee: I have recent written HSE verification, letter of “good standing “ from professional body, evidence of practice etc But I can’t find my original College of Surgical Chiropody, Cork, cert. Having the above, including success in HSE exam, do I need that cert?

From Lee: CPD certificates will not impact your application for registration. CPD will be required of you however when you join the register.

We would require the certificate, is it possible to contact the university and ask for a copy or perhaps even a headed letter which confirms the qualification title, year of award and your name.

Hi Donal, this should not be an issue. You can declare this when making your application for registration. If the team is unsure they will contact you for clarity and guide you on how to proceed.

Can documents be notorised by a UK solicitor?

Yes, that is perfectly fine so long as we can identify them by stamp, clear name etc

This refers to the statutory declaration. Please ensure the person certifying is signing in the correct place on the last page.

For proof of operational business address, I have a bank statement that has my name on it but not my business name and it has my personal address on it. I have a supporting cheque book with a corresponding account number that designates it as a business account. Is that ok?

I think record keeping is relevant

As long as CPD relates to the profession it shouldn't be an issue. If you send them in a member of the team will review and if not ok they will contact you to clarify it or see if you have anything else, otherwise they will not be in contact and just put it forward.

If i'm carrying out supervised practice in a private practice while doing return to practice am I covered as i wouldn't have a complete CORU registration yet...

Yes you can carry it out as you are under supervision of a registered private practitioner.

Do they need the full accounts or just the front page with the info summarised?

Just the front page, it is just to verify it is evidence of your private practice
