From Lee: Regarding name variation: This should not be a problem, you can declare this when making your application to clarify the situation. If the team are unclear they will contact you for clarification. It is a common occurrence and it should not have a negative impact.

From Lee: CPD certificates will not impact your application for registration. CPD will be required of you however when you join the register.

We would require the certificate, is it possible to contact the university and ask for a copy or perhaps even a headed letter which confirms the qualification title, year of award and your name.

As long as CPD relates to the profession it shouldn't be an issue. If you send them in a member of the team will review and if not ok they will contact you to clarify it or see if you have anything else, otherwise they will not be in contact and just put it forward.
