Try and build rapport first or make a connection first. If they have a navy hat, ask where they served. If the have a Grand Canyon shirt ask if they went. Or instead of being blunt, build up to it or softener your approach. Wth the financial assistance application build up to it by saying something to the effect of "you've been to the doctor a lot, that can get kind of expensive. do you have any concerns about finances."

Explaining why it is important, as well as giving them the privacy to fill it out on their own. When I worked in the clinic, I would go through it to explain it, then ask them if they would like me to step out. I made a point to not look at it in front of the patient, just thank them for filling them out, and that I would give them to the physician. That way, we never lost the trust connection between the two of us. They always knew that this was standard forms that needed to be done every X amount of weeks/months

If a screening is positive (ie. PHQ9) we do have paper copies of PHQ9 and GAD available if they don't want to answer the questions with the nurse as we may know them personally and then they can discuss it further with the provider.
