He stood up to be on the same level as the patient
he seemed to sympathize with the patient and said it was a long way and made it seem like everyone has trouble finding it. Then wanted to make sure he knew where he was going. Included a map and let him keep it.
took time to explain and review directions with patient being polite and patient
He didn't seem as though he was distracted or in a hurry.
Maybe it would have helped to already have the written instructions ready to go when she reviewed instructions so the patient had something to look at while getting the verbal instructions
I would have had the handout or instructions right there and reviewed it and highlighted things while in person with him. I would also have said both the names of drug Glucophage (Metformin) as people get confused by different name. I would also give a chart for medication administration of times.

chunk and check more effective repeating it helps people remember , going over multiple times in pieces is better
3 minutes, but if felt longer
I think this is where care coordination is a nice option to help bridge some of these gaps
setting up training with staff with each other to use these techniques then start using it with patients :)
Nice point Nicholas we do this with patients asking to call if problems or questions arise after surgeries
