Does the hospital need to follow up with EI after discharge to consider the “loop closed” ?

Unless they carry our insurance it is hard to find this data as to whether the families followed up

CLosing the loop might be a good place to use some of the opioid settlement funds: POSC coordinators who could do this

Here are the key interventions from the PA PQC SEN driver diagram related to appropriate follow up care: Refer SENs to appropriate follow-up services prior to discharge, including but not limited to Early Intervention (EI) Services, lactation support, and home visits, and close the loop on those referralsFollow-up with outpatient providers to ensure that the family care plans are adopted and engagement in outpatient careFollow the dyad for up to 15 months

. One barrier we have is our providers hesitate to start medication for moms not knowing how long it will take for moms to get in for follow up regarding depression . I’m curious to hearing what other organizations do regarding starting medications in the hospital .
