Home As The New Care Setting: Are You Ready?
Many ACOs find that delivering care in the home provides a lower-cost and more comfortable setting for patients, and can help people with complex and chronic care needs to manage their conditions in a lower-acuity setting.

How do you activate new care-at-home models and deliver measurable quality improvements in a cost-effective manner?

We’ll review four strategies to help ACOs begin to transition to home-centered care:

1. Look beyond traditional Medicare-cover services like skilled nursing and home health care. Meal delivery, transportation and home modification are examples of some of the services needed to successfully manage care.

2. Activate technology to "manage by exception" and support complex and chronic condition patients in the home setting.

3. Increase patient engagement to address health equity and social determinant needs.

4. Leverage home-based data to identify opportunities for proactive interventions.
