Leveraging Multi-Payer Partnerships with Community Care Hubs to Build Equitable Health and Social Care Ecosystems
Across the healthcare and social care landscapes, there is common recognition of the importance of addressing health-related social needs (HRSNs), such as food insecurity, housing instability, and lack of transportation. To better align social care delivery with healthcare delivery and financing, health plans and systems are partnering with community-based organizations (CBOs) to coordinate and deliver services that address health-related social needs. Increasingly, these arrangements are coordinated through a Community Care Hub on behalf of a broader network of CBOs.

Please join the Partnership to Align Social Care on December 6 at 2:00pm ET for the final webinar in a three-part series on the important role of Community Care Hubs in promoting an equitable health and social care ecosystem.

Attendees will gain:
• Deeper knowledge of the history and evolution of Community Care Hubs and their value proposition
• Insightful perspective from CBO and healthcare leaders regarding the impact of leveraging multi-payer partnerships to build equitable health and social care ecosystems
• Stronger understanding of opportunities to blend and braid multiple funding sources to support Community Care Hub capacity and service delivery
